1) Copy of your last TWO pay stubs or pay advisements.

2) A COMPLETE (30 -day) copy of your most recent bank statement . Showing the Account Number. MUST NOT be older than 30 days of application submittal date. (The submitted complete bank statement must reflect a good transaction history with no outstanding NSF items)

3) A copy of your most recent phone bill. (Phone bill does not have to be in your name, but MUST show your current address and phone number on it.)

4) A copy of your personal check, marked VOID across the front. (We use the check to verify the routing number and that the account number matches the one on your bank statement)

5) Copy of Federal or State Issued photo I.D. (Driver's license, etc.)

NOTE: Sign your name on each page you fax to us so we can easily match them with your application and speed up the approval process for you.

Upon completion of application and clicking I accept, you will have an agreement signature page appear, that you will print, sign, and fax along with the required verification items to our toll-free fax number 1-877-369-6702.

As soon as these items are received and processed, a customer service representative will contact you of your approval. (normally within one hour)
